USA Ultimate Waiver

 PLEASE NOTE - Some screenshots below are from the old UPA website, but the functionality is still the same!

 In order to be cleared to play, please complete the following steps below.  If you need help with a step, click "show me" to see a screenshot of where you should be looking.

1) Go to https://members.usaultimate.org/members/login.php and complete either step 1a or 1b:

1a) If you played in PHUL, Summer League, or have played in a UPA event before, sign in to your existing account.  If you don't know your username or password, or if you have never signed in to the online account, click "Membership Information" to access your member account. Skip to step 4. (show me)  

1b) If this is your first time playing and you have not played in a UPA event before, you need to create an account.  Do this by clicking the link that says "Brand New Member Signup" (show me)

2) Fill out all required information and click "Create Account"

3) After creating your account, you will have to wait approximately 20 minutes to be assigned a UPA ID number.  You will need to wait 20 minutes for the UPA's system to assign you a UPA ID before you can sign your waiver. (show me)  Do not go on to the next step until the red box telling you to wait for your UPA ID goes away, and your UPA ID is displayed on the screen.  You may need to reload the page or log out and log back in for this to work.

4) Your waiver status will be "Incomplete".  Click the option that says "Sign Online" (show me) and follow the instructions on the next screen (show me)

5) After you have completed this step and your Waiver Status says "complete" in green, you're ready to play in Summer League (show me)

6) Optional: If you plan on attending other UPA events such as States, YCCs, or the Club Series, you can purchase your annual UPA membership by clicking "Pay Online for 2010" and following the payment instructions (show me).  Although this is NOT required, it is required for many other events throughout the year and provides other benefits.  See www.usaultimate.org for more information on the benefits of becoming a member.
