  3. Sign and date in section D
  4. Make out a check for $44 (+ $1.50 for each extra registration card) made out to Community for Pittsburgh Ultimate (CPU)*
  5. Send completed application and payment to:

* of the $44 paid to CPU, $20 will be used to cover the $500 set up charges CPU paid to the State of Pennsylvania to order the plate design.
